Cannot add fields to tab in entry editing form
How Do I?
Fields have deleted themselves from a tab on our site’s entry editing form. The fields exist in the fields list on the right, but rather than having an open or closed eyeball, they have the text Delete displaying.
Most of the fields are created channel fields, but one of them is Author, which I understand is a system field.

If I understand what’s going on, it sounds like the icon isn’t showing so you’re seeing the alt text. See the attached for what my eyeballs look like if I disable images- you just see the ‘Delete’ text.
I’d check your cp theme url and path (though if that was bad, I’d expect you’d be noting quite a few more issues). And ‘Inspect element’ on one of those deletes- it should point to an image a la
