Unpopulated fields in Freeform Pro email notification

Please excuse me if this is way off topic since my question involves a third party module.

I have never deployed or administered (or even heard of) ExpressionEngine up until this point and need to get a site functional. I’m not a web guy either so my perspective comes from the technical side of things.

The site is developed in EE (I assume version 2? copyright is 2014 in the config.php)

It is using the Freeform Pro module and I am dealing with some odd behavior.

There is a contact form that was sending a horribly formatted admin notification. There were fields missing, breaks and returns all jacked up, some fields mismatched, etc.

For the mismatched fields it was an easy fix; the template was done poorly and had the same form fields used multiple times for the wrong data. Fixed fields in the template, saved it and shabam - no more mismatched data.

Took care or the bad formatting by creating an HTML notification template. Ok looks good, cross off the second issue.

Now the one that I am stuck on (it’s also nearing 5:30 and I give up for the day): There are two textboxes and two combo boxes. I have checked the form fields in Freeform, I have verified the template’s use of these fields, I have verified the channel configuration how the form is displayed, etc. Everything looks fine so far.

Any ideas where else I should look to get this sorted out?
