Help to make newsletter with/email activation++

Hi!
Have made this form: http://www.planportalvestfold.no/samhandling/nyhetsbrev/
Need some help here :
- When a user hit the Button, he/she must get an activation email. So when the user activate the activation url, he come to this form (picture illustration)
If user exist, the form (see picture) fill out what the user have reg. the last time..
If user want to make a chane, the user must activate the activation url he/she get in the email.
Please help.

When I want anything more complex than a simple email newsletter, I opt to use MailChimp and the fantastic SL Mailchimp Subscribe. Mailchimp makes it really easy to integrate and manage mailing lists, with really good stats tracking and anti-spam tools. SL Mailchimp Subscribe automatically registers people into Mailchimp lists when they register on your site.
