"Add Another Answer" Not Working with LG Polls

Hey there. I posted here a week ago about this issue, but because the thread is marked “Solved”, I don’t think anyone read it.
I could really use some help getting the ‘add another answer’ function to work. As far as I can tell, the scripts are linking where they should be and I followed the instructions for installing add-on explicitly. My ee install is in a subfolder, but all the paths seems to be pointing where they should be. So I must be missing something.

I had the same issue with this AddOn and had to hack into my database to add the answers. Has anyone had a similar experience. Also the instructions for LG Polls are bit lacking just in the display section. Hard to get the display working with just not quite enough explanation/instruction.
