Problem with Wiki Table of Contents

I just tried out the Wiki Table of Contents plugin, and I am not quite getting the expected results: http://www.westeros.org/BoD/Profiles/Viserys_Targaryen/
The first three headers come out right, but then something goes wrong.
The plugin version is 1.2 and EE is 1.6.8

A little bump for this. Should it have gone in another forum since this is a first party plugin?
The page in question has been changed somewhat since I posted this, but the basic example holds. It seems to be failing to nest the list properly as soon as I add more than one h3 header.
