Removing /?/ from URLs

Webmaster Tools is highlighting duplicate content:

URL A/ domainname/section/page

URL B/ domainname/?/section/page

Any ideas about how to fix it, or what could be causing it?

index.php is removed from urls via .htaccess by the way.

If I could redirect all /?/ pages to / …it would be brilliant.
