Help me get Coda's Live Preview working with EE2

I really am having some issues getting the live preview to work in Coda with EE site (saved as files). Have read and worked with the .htaccess file but must be missing something. Would really appreciate a Coda ‘guru’ helping me out a bit.
In return I would be happy to post in detail my resolution for the community.

Would you care to share what you figured out? For the past year or so, I was following the suggestions in this blog article: http://ellislab.com/blog/entry/live_preview_with_coda_and_other_ides
But lately, that method hasn’t been working for me. I’d be happy to hear what you figured out.

The key point I missed once that the live preview won’t initially load correctly and you need to manually “refresh” it in order for it to work. - So if doesn’t look like it is working - click in the preview url bar and hit enter (to refresh the page).
That worked for me!
