How does the multiple site add-on work, do you purchase this one for all the sites you plan on using EE with, or do you need to purchase it for each site after the first one you plan on hosting?
I assume EE does not manipulate apache configuration, so I must assume there is a separate EE installation per virtual host (i.e. domain name) when using multiple site option?

Are the sites that you wish to run all owned by the same person? That is a requirement for the Multiple Site Manager. If they are not, then you will need full licenses for each site.
As for purchase, you would purchase one copy of ExpressionEngine and the matching license for the Multiple Site Manager which gives you 3 Sites. You can find the pricing for the MSM, and Sites beyond the initial three, at the Multiple Site Manager page. The requirements and documentation are here.
