Is it possible to have all commenting on blog entries occur in the forum. So instead of having a separate ‘comments’ section, when someone chooses to comment on a blog entry they are taken to the forum where either a new post (with a link back to the comment) is created if it’s the first comment / post or if it’s the 2nd or later comment / post it’s taken to the original comment.

Yes, you can setup a blog so that people post comments to a forum thread associated with a weblog entry. In fact, this is how our own ExpressionEngine Blog works. If you look at our blog and click Discuss, it takes you to a forum thread. A new forum thread is automatically created in the EllisLab Blog Discussion Forum Category per blog post (not per comment) and the link back to the blog post is generated automatically from the Publishing Screen.
