i built my site on the FREE version and now ready and want to add the license
Do I need a new EE install or do I just add the license #?
How do I make sure I do not loose the theme , settings etc?

but according to this it is a complete install

See https://ellislab.com/expressionengine/user-guide/installation/upgrade_from_core.html for updating from Core.
If you are using the *same* full version as Core then you only need to update the /system folder and not run the Update wizard.
If the full version is later than your core version then just follow the normal update instructions and run the update wizard.
As always take a full backup either way.
