I have a search form—but need to be able to designate the custom channel field that each search field searches:
* A keywords text field would search Title and three specified custom (text) channel fields
* A dropdown would search a specified custom (text) channel field
* And another dropdown would search a specified custom (text) channel field
Is there a way to do this natively? Or is there an add-on that will help me accomplish this?

Probably not out of the box with the stock search because you’re asking for something quite specialised.
Low Search has the ability to restrict searches to certain custom fields so worth looking at that - http://gotolow.com/addons/low-search

Does Low Search allow me to specify which search fields correspond to which custom channel fields?

Well in my totally unbiased opinion you could also try Reefine. It uses lists of links to filter custom channel fields. It doesn’t natively support dropdowns at the moment (I’m still working on this) but you can use some javascript magic to get it to work with them.
