Using MSM, some server and workflow questions

I’ve finally got a nice local, development, live workflow going with MAMP, Git, and Beanstalk. Works great for the three of us who work on the various sites.

However, that is all about to change. A new project has us migrating some older sites to EE 2.0. These are part of our same organization and is an ideal candidate for MSM (multisite for those of you wondering). MSM is the way we have to integrate this. That is a requirement for the project.

    Questions:
-Currently using NSM bootstrap with great success. MSM more or less tosses that out. Any bootstrap/MSM solutions out there? I’m going to be trying one a local dev put together, but always want to cast as wide a net as possible.

-We currently don’t have a true development server. dev.example.com lives on the same server as example.com. In order to have our system folder above web root AND have a dev and live version they will need to be named different.
    A) Is there a way to edit the bootstrap to allow for this?
    B) Alternatively, could I just ignore my index.php and admin.php files in Git and set those manually?

This is the first time I’ll be using MSM and as great as it is for the product (3+ sites sharing member data and content) it sure is a downer for our workflow! Any ideas from those of you who have been here before?
