When I want to add an entry or edit an entry the file upload box is always shown below the footer (see image).
If I want to add an image or file, the pop-up window for the existing files is shown as expected.
If I choose ‘browse server’ and then ‘upload file’, nothing happens.

So if I want to add an image or file, I first have to go to the bottom of the page to upload an file. After that I can select the file (with browse server). It’s not a normal workflow and it’s a problem for my client. After all, this isn’t suppose to happen…:-)

The problem did exist in EE2.4 and remains after updating to 2.5.2.
On this server I run also other EE-installations and the other sites don’t have the problem.
The problem exist in safari and chrome on the Mac. (Did not test it in windows)

I use wygwam as text-editor. Latest version.

Thanx to m0nkey I know the problem: the template variable accessory is causing this problem. I uninstalled it and now everything works fine.
