Error with exp_security_hashes table, site down

    Over the weekend, one of our sites was not loading and throwing the error:

    A Database Error Occurred
    Error Number: 1062
    
    Duplicate entry ‘194969’ for key ‘PRIMARY’
    
    INSERT INTO exp_security_hashes (date, ip_address, hash) VALUES (UNIX_TIMESTAMP(), ‘74.242.XX.XX’, ‘800a13sfe1f20ab4bbba2b8fe0901e76ef00a06a’)
    
    Filename: libraries/Cp.php
    
    Line Number: 658

    We were able to clear the table mentioned and the site came up again.

    Any ideas what might have caused this error, and what we can do to prevent the entire site from coming up in the future if this error occurs again?  The site is running EE 2.2.2, but we didn’t see any mention of this error in the changelogs for the newer versions.

    Hello swg,

    I am sorry to hear you are running into this problem.

    I searched the bug tracker and the forums a bit. Nothing out there on this that I can tell.

    Was there any maintenance over the weekend being carried out by your hosting provider? Like maybe the database was restored from backup for some reason and that caused the duplicate entry?

    I do not think this was related to something that ExpressionEngine was doing at the time. It feels like a database issue with the double entry.

    Would you be willing to check with your provider?

    Let me know what you find and we can go from there.
