Did you click the “Select/Deselect All” checkbox before clicking “Remove Layout”?

yes. And since the boxes weren’t automatically selected the first time I tried it I also tried a couple of alternate times clicking each box individually.

Which version are you running? In 2.2+ the button is labeled “Reset Layout” but your screenshot shows the old “Remove Layout” text. Also, try disabling extensions to see if that makes any difference.

Thanks for the tip. We are running: v2.2.1 - Build: date 20110705 and disabling the extension didn’t solve the issue. Though it is curious that I am seeing the “Remove Layout” text

no, I have never been able to do a manual removal (I made multiple attempts)

Sorry this isn’t resolved for you yet, I have some additional steps for you to try.
It appears you may need to TRUNCATE a table. I’ll outline those steps for you:
1. Essential! Make a backup of your site files and your EE database BEFORE attempting this.
2. Navigate to Tools-> Data and then click “Database Query Form” from the buttons over on the right-hand of the CP page.
3. In the query form, issue the command “TRUNCATE TABLE exp_layout_publish” with out the quotes.
Please let us know if that resolves your issue.
