Edit: Solved by manually removing the extension from the database.

I’d like to add javascript to the control panel using an extension that is bound to the cp_js_end hook. However, the custom javascript can’t be loaded because the url that includes it on all control panel pages returns an error. Here is the url generated by EE:

Here is the error:

Unable to load the following extension file:
ext.dynamic_member_fields.php

I can’t find a “dynamic member fields” extension installed. Any clue where that comes from?
