Recently went through this process myself. Do not use Microsoft for any CSV work on a Mac as it is atrocious.
Use OpenOffice or NeoOffice for the Mac. This will make sure you have the correct line endings etc.

Another online tool I came across recently if you want to bypass the conversion tool altogether is Mr.Data Converter http://shancarter.com/data_converter/ The tool is excellent and you will have your XML in seconds
Only thing to remember is name your Column heading sin your CSV to the same field name as your member fields
