It means that when you receive a notification, you won’t receive any more until you have visited the thread.
Without smart notifications, you will blindly receive a notification for every reply.

Ah, cool… Now I know why I’ve been getting unexpected updates (or lack thereof) on the CartThrob forums. Cool feature, but guess I’ll need to add a bit of a description to it.
