The step I suggested won’t retroactively fix the broken site; you’ll need to delete the site and try again with the new file in place. What you’re seeing is a symptom of a deeper issue, which is that an error is occurring in the middle of creating a new site, leaving that site incomplete.

Hello Brandon,
Just updated my installation to latest versions of EE 2.1.1 build 20101020 and MSM Version: 2.0 Build: 20101018.
Changed email_data.php in the process.
Well I don’t have the comments_opened_notification_title error anymore (new function en email_data.php) and the login problem is gone.
I still have a problem though.
When I try to create a new site, I stumble upon some php errors and a database error.
See screenshot attached.
It seems to create the new site though.
The new site is there, with the channels, templates, etc.
I didn’t investigate much further, since this database issue seems frightening.
Can you have a look at it ?

I suspect some other files need updating. Did you also replace your themes/cp_themes folder with the latest versions during the upgrade? Give that a try.
