I’d like to have an opt in form where people fill in name, address, phone, and email. Then have that saved in EE somewhere.

So basically, I don’t want them to do the hassle of actually signing up as a “member” in EE(username/pw, etc), but I also want to store more than just name and email (which can be done w/ the Mailing List manager). How is this done?

Basically, you’ll have to code such a forum yourself. Do take a look at Solspace’s Freeform, it should get you there most of the way.
