    This cropped up suddenly and without and for no discernible reason (i.e.: no recent changes to hosting, or CP settings).

    A PHP Error was encountered
    Severity: Notice
    Message: Undefined offset: 2
    Filename: libraries/Functions.php(659) : eval()'d code
    Line Number: 5

    EE 2.4.0 - Build: date 20120123

    Does not appear in the CP, just the index page of the site (no internal pages either).  I found similar issues in the forums, but nothing that helped to fix the issue.

    Thanks for your help!

    Hello Will,

    I am sorry to hear you are running into this problem.

    Do you have anything new on your homepage? Some PHP code recently added?

    Any recently added Add-Ons?

    Are you 100% sure that no changes have taken place on the server in regards to PHP?

    Please let me know.

    Cheers,

    Hi Shane,

    I’ve resolved this.  It was a PHP issue from an embedded template.  My fault.
