Tried updating my local install of EE from 2.2.2 to 2.3.1 and at the point I went to sitename.com/admin.php I got the CP homescreen with the message:
Patch unsuccessful!
This patch cannot be applied to ExpressionEngine versions older than 2.3.0.
Please do a full upgrade to version 2.3.1 or contact tech support for more information.
Previously in the process, following the notes on the upgrade page in the user docs, I got an error saying:
An Error Was Encountered
Unable to load the requested file: _shared/menu/item_parent.php
I then realised I had updated the themes/third_party and third party theme docs (eg devdemon and solspace dirs) dirs, but not the cp_themes/default.
Looking in the cp_themes/default dir, it only has the css and image dirs. Adding back all the folders from my backed up themes/default dirs

I then copied my themes/cp_themes/default folder from the backed up dir to the new themes dir to put that _shared and other CP dirs back in there, and I get the unsucessful patch error (attached image2)
