After upgrade, File Manager is broken

After recently upgrading to 2.2.2 build 20110801, the File Manager is broken. The “Upload” button appears positioned incorrectly and generally styled wrong. Clicking the “Upload” button just refreshes the page rather than opening the upload dialog. I’ve attached an image for clarification. I have tested this with and without the NSM Morphine theme, and it appears it is broken with or without added extensions. Thoughts? Hoping to get this resolved quickly since I’m on a deadline. Thanks for your help!
