goal: remove index.php and template group
but: I can live with the template group if necessary

mpt is the template group

Yes, but you have to know what you’re doing. In your case, you’re basically telling the server: “If file not found, try again with prepending “index.php/mpt/” So, http://www.medicalpracticetrends.com/member/login/ would become http://www.medicalpracticetrends.com/index.php/mpt/member/login/—obviously, that doesn’t work.

Personally, I find removing index.php acceptable (I do it myself), but it’s not officially supported. I strongly advise against removing the template group, however.

Your code matches that standard code we use to remove “index.php”

here is our code:

RewriteCond %{REQUEST_FILENAME} !-f
RewriteCond %{REQUEST_FILENAME} !-d
RewriteRule ^(.*)$ /index.php/$1 [L]

But we do not attempt to remove the template group - all attempts have failed. We have turned template group naming into an art form (so it ads value if possible).

Check your system preferences:
Make sure that “Name of your site’s index page” is empty
Set “URL to the root directory of your site” to http://www.yoursite.com/

That should do it. Your member pages (login, register, profile, etc) should not include “index.php” in the URL..
