Branching Question Forms / Survey

    Is anyone aware of any existing solutions, or recommended approaches for creating dynamic forms that can branch depending on the answer to a question?

    This function seems to cross over several areas of functionality including, Form creation, Survey creation, Polls and possibly Quizzes.

    The general functionality I’m looking to accomplish is as follows:

    - Needs the ability to ask branching questions
    - Depending on the answers to the questions, a ‘response’ message will appear at the end with instructions for the user
    - The ‘response’ for the user must be able to be emailed upon request, or downloaded as a PDF

    Example:
    question 1: How old are you?
    option 1: Younger than 50
    option 2: Older than 50

    If response on Q1 = option 1, Question 2:
    question2a: How much coffee do you drink most days?
    option 1: less than 3 cups
    option 2: greater than 3 cups

    If response on Q1 = option 2, Question 2:
    question2b: How much orange juice do you drunk most days?
    option 1: less than 2 cups
    option 2: greater than 2 cups

    Final Response:
    If question2a response: option1 - repsonse1: “great you are healthy, here is your certificate”
    If question2a response: option2 - response2"stop drinking so much coffee, go to this link and get help”
    If question2b response: option1 - response1:“great you are healthy, here is your certificate” (Note same response as #1)
    If question2b response: option2 - response3 “stop drinking so much orange juice, sugar is bad go to this link and get help”

    I’m fairly sure this type of branching is available in some of the form modules, but the dynamic responses in the end is not something I’ve seen.

    Would possibly need to custom code in codeigniter the ‘Final Response’ piece?
