Attempting to update an EE2.1.4 to EE2.2 update and getting the following error during the upgrade process:

A Database Error Occurred

Error Number: 1061

Duplicate key name 'last_activity_idx'

CREATE INDEX last_activity_idx on exp_sessions(last_activity)

Filename: updates/ud_220.php

Line Number: 82

Any ideas? I updated all my add-ons to the latest version before attempting the update.

OK, running it again has apparently updated the install.
Seeing several bugs though:
My site’s index page is now showing:

Meanwhile in the control panel the pagination at the bottom of the EDIT screen is simply gone.
Trying to access any custom field gives me a similar error:

OK, some progress, uploading the safecracker for 2.2 folder into third_party solved most of the error messages. See http://ellislab.com/forums/viewthread/192263/
Contacted Litzinger via Devot-ee for the snippet sync errors.

Still missing the pagination on the edit screen though.

Rolled back to the corporate theme, and disabled template variables and leevi’s override css to exclude any errors caused by these.
I ended up disabling the snippet-sync extension for now, apparently the error message causes some kind of js error which causes the pagination to not be loaded, it’s back now.
