I get this error when I active a pending member

What version and build of EE are you using? Any extensions?

EE1.6.8 latest build
using several from lg and Solspace: Solspace UI, software-update,User,Freeform, lg-better meta, lg-polls, lg-sitename,lg-addons
This seems to have just started after upgrading to Solspace User 3.2.2 a couple days ago

My suggestion would be tu re-upload a fresh set of all your files. Also try without “User” if you can.

Are you suggesting extension and module files, EE system files or both.

This seems to have just started after upgrading to Solspace User 3.2.2 a couple days ago
This looks like a Solspace User issue. The latest version of the module may require you to have EE1.6.9 or your User upgrade may not have gone right.
Either way I would suggest contacting Solpace support
