Errors when Uploading images in an entry

I am having an issue with uploading images into an entry. I go through all the steps (the image actually gets uploaded to the server) but I get stuck with the following on the screen with no way to actually add the image to the entry.

I recently changed my domain name but uploading images into an entry worked before I changed the domain name. Any ideas what could have happened with the domain change to cause this and how do I fix the problem?
Would you happen to have been doing some troubleshooting and added a die() , var_dump, etc statement anywhere in the core?
If not, I would start by checking out the dev tools Network tab in your browser while trying to upload and check all the responses from different calls that are made.
Andy, I haven’t been doing any troubleshooting in the core.
When I used the dev tools in my chrome browser I see the file where the error is coming from. Image Attached. Any further insight would be greatly appreciated.
