Grid - Undo Remove on a image field - undo remove does not work as it should.

In the CP and a Channel Form I have a strange issue with ‘undo remove’ not working as designed.
updating’ in the CP or via a Channel Form (no conflicting JS, tried removing all styles bar the default as loaded by EE) - if the small cross is clicked to remove an image from the row and then ‘undo remove’ is clicked, the image reappears as it should.

However, when submitted and updated, the image is not saved. Upon editing the entry again after that action, the file field is blank.

Any ideas?
