As per this bug report, newly-created file upload locations do not appear in the “upload file” overlay on the Publish / Edit page. Logging-out and back in resolves the issue.
This is still an issue with the most recent build.
OS : Mac OS 10.6.4
Browser : Safari 5

I’m still unable to replicate with the same client-side setup. Can you erase your /themes/cp_themes directory and repopulate it with the one from the download package, clear all your browser caches and try again?
If that doesn’t work, there may be some server-side caching going on.
