    Where is sample code for entry and comments on the same page?

    I can find each in it’s own page, but am having trouble putting comments under the entry on the same page.

    If you want to have a list of entries, all with their comments on the same page, this is not directly supported. There’s a workaround described in the wiki, but you probably should keep comments on single entry pages, where they belong.

    I’m looking for a single entry with comments and comment form on the same page, rather than on a comment page template.

    The comment page template really is such a single template: it contains the entry, the form and the comments. Just name it “permalink” or whatever instead of “comments” and link to it directly from your index page.
