World countries, regions.. when use category?

Hi all,
I have to create a channel linked to continent
AND
country OR countries
OR / AND / NOT
first level
OR / AND / NOT
second level

Example:

ME linked to EUROPE -> ITALY -> NOT ROME
YOU linked to EUROPE -> ROME (only)
HE linked to EUROPE -> ALBANIA -> * and EUROPE -> SPAIN -> NOT MADRID
SHE linked to AMERICA -> BROOKLYN (only)
IT linked to AMERICA -> ALABAMA -> *

How to use category in this case?
After this I’ll have to list ALL EUROPE entries, ALL ROME entries (1!), etc..
Any suggestions?
