I have what appears to be the exact same problem. A patched-up debian/etch installation with php5/apache2/mysql5 and the latest version of EE. Everything is fine until page5 - a header loads on the page - no tables are created and my config.php file is zero bytes.

I’d love to get this solved soon - any thoughts?

Hi Dean, can you first make sure that ‘display_errors’ is enabled in the PHP environment?  Introducing a syntax error into the install file to double check that errors are being displayed would be good, too, for instance by removing a semicolon from one of the lines at the top of the file like so:
