I’m just curious as to why it’s adding that additional ?/contact-form/

Is that an EE thing? A Redirect Helper Lite thing?

Could be an EE thing or an htaccess thing. Redirect Helper Lite builds the destination URL from EE settings.

Do you have “Force URL Query Strings” turned ON? You can find that under Admin >  System Preferences >  Output and Debugging Preferences

Wow, I don’t think I’ve ever touched that setting, but switching it to Yes seemed to solve the problem.

Ok, maybe I spoke to soon. The previous example that I gave which redirected to an external site worked fine. But when I try to do an internal example, it doesn’t seem to work right:

/blog/news/transition/ -> /blog/transition/ ends up redirecting to /?/blog/transition/

Also as a note, I am using the LG .htaccess generator with all of the default settings.

For anyone else reading, Trevor was able to turn off LG .htaccess and re-enable it followed by turning OFF Force Query Strings. Now Redirect Helper Lite works as expected.
